The top of form has boxes for the donor’s name, contact information and address. There’s also a box indicating whether or not to make the donor’s contribution public or not.
This donation receipt is completely customizable upon request but you can further custom fit this form to your needs by choosing consecutive numbering and ink color! We offer black and red consecutive numbering which prints underneath the title of this forn “charitable donation receipt.” Black and blue ink is free but you can upgrade to full color logo which goes at the top left hand corner of the book.
