This 8.5 x 5.5 delivery invoice form covers all of the areas in order to charge clients for the delivery of packages or pallets (weight, number of packages, sizes, delivery time, drivers name and more). Like most of our forms, Form 761 is available as 2 or 3 part carbonless paper sets.
This form was designed to include: separate sections to include three distinct addresses (Bill To, Pick-up From and Deliver To), boxes to fill in order details (Date, Order Number, Weight, Driver, etc.), lines to include a description of the shipment and boxes to indicates all charges.
