Load Ticket Invoice Form is a full page trucker form that records load times and weights. A full page form allows for bigger boxes and more space that will make writing and reading job details easier. Billing address, location and material boxes are located at the top. The trucker will record their start, stop and total hours in a large box on the right. Below the location and hours, there are 21 lines for load times, ticket numbers and weight. In addition, there are 5 more lines available for calculating totals. These lines consist of rates, load count, total ton, total hours worked and amount due. Authorized signature and operator signature lines are near the bottom of the form. Legal terms have also been provided, however, our customers will be able to send their own terms during the proofing process.
