This small custom work order form is 8.5×5.5. The company information prints in the top left. A box to the right holds the order numbers, dates and who took the order. The first table is where you record the customer’s bill to, address, city, job location, contact information and description of work. There are three check boxes for “day work, contract or extra” for distinguishing what kind of job this is. The center table is where all the material is listed. There are columns for quantity, price and amount. Three lines for hours, labor and amount have been added so that your customers know what they’re paying. Your technicians can then add the total amount of materials, labor and tax. We’ve also included an additional box if you need to add up any further expenses.

Our custom forms are available on 2 or 3 part NCR paper. This means that the top copies all the way to the last part. Further customize your work orders by choosing consecutive numbering, books or upgrading to a full color logo.
