We sell Trip Cost Report Envelopes that print with your company information and logo. These 9×12 white envelopes are fully customizable to your business needs. This means that during the proofing process you can request additional changes to the text and layout. Additionally, if there is any part of this form that will always have the same information we can have that pre-printed on the envelope. For example, if the Origin is always the same we can pre-print that address. Our designers will spend as much time as needed to ensure that your envelopes fit your business model.

The top left of the envelope houses your company logo, name, address and phone numbers. Across from that, the words Trip Cost Report prints in bold letters. This helps your employees know what this envelope is for. If you so choose, consecutive numbering prints underneath the envelope title so that you can keep better track of all documents pertaining to a trip.

The next section holds all of the driver and truck information and signatures as well as the dates. Beneath that is where the truck leaves from and the destination.

A table in the center is for route and mileage. Under route, there are columns for date, state and highways used. The mileage area covers odometer readings and total miles. The final table covers all additional costs like fuel and maintenance charges.
