The following elements are customizable upon request. The top of the form has a line for the company name, truck number, trailer number and dates. There are two lines for two drivers. Your company information prints on the top right. There are 7 trip address lines. The larger table holds all of the information the drivers accumulate during their trip. This information includes date, state, odometer readings, highway used and state miles. The bottom table holds the following details: date, number, gallons, cost, name, city, state and method of payment.
