The appliance invoice form is an extensive invoice record of services performed as well as a legal document between customer and client. There are 8 lines at the top left for listing part numbers and descriptions. In the middle of the form, technicians will have an area to write their professional diagnosis of the issue then right below they can describe the work performed.
