During the proofing process, customers are able to request additional changes to their forms. These changes include making revisions to the following elements.
The top of the form handles the service location, charge to and appliance information. There are boxes for customer name, job location and telephone. These same boxes are then repeated below in case the billing information is different from service location. The A/R# and job numbers are also in this section. Appliance type, make, model, year, serial numbers, warranty and problem are all under the appliance section. The center table holds quantities, service rendered or parts used, price for each and the total.
The bottom of the form has a table for service rendered. This included the technician name, date completed, payment methods, labor time and hours. There are two signature boxes. One for the technician that takes the payment and another for the customer. The bolded boxes on the bottom left have parts, p.s.t, labor, service charge, subtotal, g.s.t, h.s.t, total amount, amount paid and balance.
