The roadside tow service form is an 8.5×8.5 version of the 615 roadside towing service invoice. Bigger form means more space to write in, more room for remarks, and a larger payment information box. Everything on this form can be customized to your company’s needs. Tow services and reason for towing check lists do come with the form, but all of the information you see can be changed to fit your business.
Notable form features: ample room to write down customer information as well as vehicle information (location, year, model, Vin #, Tag #, etc.), lines to indicate the mileage, a section to detail the length of service, a decent area to write down the description of any work, a check-off section to indicate who requested the towing service, an area to note the reason of service, space to detail any additional equipment used, a box to note the storage information, a section for method of payment including space for a credit card number, a box to write down any remarks, a section for authorization signatures and boxes to include charges for all labor and fees.
