Service Order Claim Check prints with your company details and logo on the top of the form. Your company name and phone number also prints on the tear-off stub. Consecutive numbers print in black ink on all three sections so that your orders will never be mixed up. Invoices with detachable claim checks are a must have for repair service companies! This product increases productivity and accuracy.
The service order includes customer information, equipment model information, payment type checklist, type of invoice and the appropriate authorization signatures to prevent loss of revenue. A table for listing parts, quantity, price and description takes up the middle of the page. There are 14 lines here. Below that is a large area for additional comments. The total boxes include checklists to denote where the service is taking place and if the items need to be picked up or delivered.
This form does have two hard cover manila stubs that detach from the invoice.
